Albert 'Al' Sidney Johnston Underwood was the son of David Moses Underwood and Sarah Elizabeth Foster. He was named for General Albert Sidney Johnston. Al's father had served under Genernal Johnston during the Civil War.

Al married Martha 'Mattie' Caroline Church in Barren Co, Ky on May 15, 1889. They had 7 children.

His obituary dated Aug 20, 1954 states he 'died yesterday' (Aug 19th), however the administrative office at Fairview has that he died on Aug 4th and that his wife Mattie died on Aug 20, 1954 (she actually died in 1966). His death certificate has Aug 4, 1954 as the date of death, and Aug 6, 1954 as the burial date.

Al is buried in the same section and lot as wife Mattie, and son Watt.
