Evelyn worked at St. Joseph Hospital for over 30 years. After retiring, she became a full time homemaker.
Evelyn leaves behind her beloved husband of 57 years, Don Chism, her children, Dennis and Eileen Chism, Betty and Lester Phelps, David Chism, Chris Chism, and daughter-in-law Rita Chism, and grandchildren, Marcy Chism, Kristie Ray, Jenny Titus, Travis Phelps, Jeff Chism, Julie Hagermann, Jennifer Bice, Jessie Bice, and Cassie Chism .
Evelyn was preceded in death by a son, Bill Chism, grandson, Barry Phelps , and by her sisters and brother, Mary, Olivia, Aurora, Ruby, and Jack.
Friends are invited to attend graveside services Tuesday, September 28, 2004 at 10 am at Sunrise Cemetery, Fortuna, with Fr. Jim Trent officiating.
Arrangements are under the direction of Goble's Chapel Fortuna where friends may call Monday from 7 to 8:30 PM.
In lieu of flowers, the family requests memorial contributions to your favorite charity.
