Declaration for Pension
State of Iowa, County of Polk ss. On this 11th day of Jan A.D. 1922 personally appeared before me, a Notary Public within and for the county and State aforesaid, Silas W. Reynolds, who being duly sworn according to law, declares that he is 77 years of age, and a resident of Des Moines, county of Polk, State of Iowa; and that he is the identical person who was Enrolled at Washington Iowa, under the name of Silas W. Reynolds, on the 20th day of February 1864, as a private, in Company I 40th Regiment of Iowa Infantry in the service of the United States, in the Civil War, and was Honorably Discharged at Fort Gibson on the 2nd day of August 1865. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height 5 feet 7 inches; complexion light; color of eyes blue; color of hair light; that his occupation was farmer; that he was born June 28 1844, at Miama County Indiana. That he requires the regular aid and attendance of another person by reason of asthma and heart trouble. That he is a pensioner under Certificate No 513970. That he has applied for pension under original No 513970. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of May 1, 1910.
