Declaration for Widow's Pension
Act of May 1, 1920
State of Iowa, County of Polk ss. On this 7th day of April, A.D. 1922, personally appeared before me, a Notary Public within and for the County and State aforesaid, Anna M. Reynolds, who, being duly sworn by me according to law, declares that she is 72 years of age and that she was born June 15, 1849, at Jefferson County, Ohio. That she is the widow of Silas W. Reynolds, enlisted Feb 29, 1864, under the name of Silas W. Reynolds, as a Private, in Co. I, Fortieth Iowa Inf., and was honorably discharged, August 2, 1865. That otherwise than as herein stated said soldier was not employed in the United States service. That she was married to said soldier May 6, 1875, under the name of Anna M. Haycock, at Keokuk County, IA, by a minister; that she had not been previously married; that he had not been previously married and that neither she nor said soldier was ever married otherwise than as stated above. That the said soldier died March 29, 1922, at Des Moines, Iowa, that she was not divorced from him: and that she has not remarried since his death. That the following are the only children of the soldier who are now living and under 16 years of age, no minor child. That she has not heretofore applied for pension. That she hereby appoints, with full power of substitution and revocation W. W. Fink, of Des Moines, Iowa her true and lawful attorney--to prosecute this claim.
