The Cadiz Record obit posted 19 Apr 1973 reads as follows:


Tollie Enis Cook, 79 died Saturday, April 14, at 2:25 pm at Shady Lawn Nursing Home. He had been in declining health for several years and a patient of Shady Lawn Nursing Home for the past three and one-half years.


Mr. Cook was born in Stewart County, Tenn., September 4, 1893, the son of the late John Cook and Arminta Spiceland Cook of Shady Lawn Nursing Home. He was married to Onie Mathis, who preceded him in death several years ago. He was a member of the Baptist Church and was a retired self-emplyed carpenter.


Funeral services were held at Goodwin Funeral Home, Monday, April 16, at 2 pm, with Rev. O D Boyd officiating. Music was by Mrs. James Tuggle, organist. Burial was in Matheny Cemetery.


Survivors, in addition to his mother, Mrs. Arminta Cook, include two sons, Cecil and Robert Cook of Columbus, Ind.; two brothers, Forrest Cook of Cadiz, and Leland Cook, Princeton; one sister, Mrs. Noble Allen of Trigg County; eight grandchildren; and two great grandchildren. One son Coy L Cook, preceded him in death several years ago.


Pallbearers were Harley Cook, Waymon Cook, Lacy Cook, Forrest Cook, Bobby Jewell and Leland Cook.
