Maiden name could also be Wyzial, Wyzral, Vyzrat or Vyzral. Last name could also be spelled "Koutnec", "Kautnek" or "Koutnek". Immigrated to the United States circa 1913. Married Frank Koutnik on January 8, 1916 in Chicago, IL. Mother of Alfred, William, Edward and Harry. Olga and her son Harry were both killed in the Favorite Disaster on July 28, 1927. 27 people were killed when the boat capsized in Lake Michigan. Olga was 38 years old at the time of her death.

Olga and her son, Harry, are listed among the drowned along with 25 other people when a steamer capsized in a wild storm gala off the Gold Coast .

From the Chicago Tribune 7-29-1927:
"Sixteen children, ten women, and one man were drowned yesterday when the "Favorite," a small excursion boat cruising from Lincoln Park to the Municipal pier, capsized half a mile off North avenue in a sudden, terrific squall. Ten of the children were boys and six girls. Twenty-five of the dead have been identified."
