Beulah died on 22 October 1918 at Winchester Memorial Hospital of "the influenza" (also known as the "Spanish flu") epidemic which was sweeping the country in 1917/1918. She was "with child" at the time of her death.
According to her death certificate, she is buried in "Carper Valley, VA" which is in Frederick County. This is an old Carper family cemetery with many unmarked graves on private property. In memory of the 100 anniversary of her death in 1918, her family erected a headstone in her honor on August 16, 2018. The cemetery can be reached by Carper Valley Road and Kenny Lane. Public access to the cemetery is between lots 241 and 291 on Kenny Lane and is approximately 100 yards from the edge of the road. There is a cemetery sign on the left side of the road directing entrance into the woods where the cemetery is.
There appears to be a close connection with the Carper family as the Kurtz Funeral Home records have recorded the death of her daughter Lena M Carlyle on 16 October 1916 being reported to them by J. L. Carper. This may possibly be Julian L. Carper who was listed on the 1920 census as head of household in which Beulah's son Ray Maxwell Carlysle was living as a "lodger" at the age of 15.
