From the transcription:
"In memory of Miss Polly Annable, daughter of M^r. Joseph & Mrs. Experience Annable, she died Dec^r 8th 1813 in the 26^th year of her age."

Farther down on this same page, Samuel Annable's death is noted to be memorilized on "Polly Annable"'s headstone, but which of two Pollys in the manuscript isn't specified, nor is their relationship.
