Mrs. Hazel Dale Lewis Faulkner, age 86, of Maysville, Kentucky, went home to be with the Lord, Thursday afternoon, August 18, 2016, at the Maysville Nursing and Rehabilitation Center in Maysville, Kentucky.

She was born November 30, 1929, in Beckley, West Virginia, a daughter of the late Elmer Dale and Nannie Belle Ratliff Lewis.

Hazel was a member of the Vanceburg Christian Church and the Vanceburg Historical Society. She enjoyed traveling and reading.

In addition to her parents, she was preceded in death by her husband of 45 years, Orah Faulkner; one brother, Elmer "Buddy" Lewis; and one sister, Helen Ruark.

Hazel is survived by three sisters, Lorene Free of Hebron, Kentucky, Carla Lewis of Florence, Kentucky, and Dorothy Hunter of Vanceburg, Kentucky; two special nieces who cared for her during her sickness, Patsy Lewis and Carolyn Kinney; and several nieces and nephews. She also leaves many other family members and friends who will sadly miss her.

Funeral services will be held at 2 p.m. Sunday, August 21, 2016, at Dickerson Funeral Home in Vanceburg, Kentucky, with Brother Kevin Cornette officiating. Burial will follow in the Lewis County Memory Gardens in Vanceburg, Kentucky.

Friends may visit from 12 noon to 2 p.m. Sunday, August 21, 2016, at Dickerson Funeral Home, 110 West Second Street, Vanceburg, Kentucky 41179.

Family and friends will serve as pallbearers.
