David was born on Indian and Weatherford Creeks in Wayne County, Tennessee. A short time later, the location became known as Cravens Mill, and in about 1850 became known as Martins Mills. He was the son of John Cook, Jr and Deborah Martin Cook. He moved to nearby Tishomingo County, Mississippi in 1844 following his grandfather's death. There he married Rebecca Elizabeth Davis in 1845. David and Rebecca moved again to Upshur County, Texas sometime after the 1860 census, and remained there for the rest of their lives.
David and Rebecca had six children, all born in Mississippi:
