"That his pain on the right brest was caused by a fall when working at carpenter work about 14 years ago and that he is also troubled with asthma and has been for a number of years and also troubled with skin disease on the left leg so that he is unable to ride on horseback in consequence of same.

"That eczema first appeared on my left leg about 15 years ago in a mild form and has been gradually spreading and becoming more troublesome until the present. I was first attacked with rheumatism about 12 to 13 years ago, which has troubled me ever since by times more severe than at other times, but increasing in severity as I grow older- and swear that the aforesaid troubles are not the result of vicious or immoral habits."
