DECLARATION FOR INVALID PENSION
State of Illinois County of Pike } SS

On this First day of May, A.D. one thousand eight hundred and eighty three personally appeared before me, Clerk of the circuit court, a court of record within and for the County and State aforesaid Wm E Smith aged 40 years, a resident of the town of Nebo, county of Pike, State of Ills, who, being duly sworn according to law, declares that he is the identical Wm. E Smith who was ENROLLED on the 17 day of July, 1862, in Company I of the 70 Regiment of Illinois, and was honorably DISCHARGED at Alton, Ills, on the 23 day of Oct, 1862; that his personal description is as follows: Age, 40 years; height 5 feet 10 ½ inches, complexion, dark; hair, dark; eyes, gray. That while a member of the organization aforesaid, in the service and in line of his duty at Alton, in the State of Ills on or about the 30th day of September, 1862, he contracted chronic diarrohea caused by exposure ie, from which he has never recovered, and is still disabled. He claims Pension on account of chronic diarrohea.

That he was treated in hospitals as follows: at Alton, Illinois by __________, Surgeon. That he has not been employed in the military or naval service otherwise than as stated above. That since leaving the service this applicant has resided in County of Pike in the State of Ills, and his occupation has been that of a farmer. That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a farmer. That he is now totally disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this claration for the purpose of being placed on the invalid pension roll of the United States.

He hereby appoints, with full power ____________ of Washington, D.C., his true and lawful attorney to prosecute his claim. That he has never received nor applied for a Pension. That his Post-Office address is Nebo, county of Pike, State of Ills.

I, J.H. Cromwell, am a practicing physician residing at Altona, Knox County, Illinois. From June 16, 1903 to Aug 30, 1908 I resided and practiced medicine at Nebo, Pike County, Illinois during which time I was frequently called upon to professionally treat William E. Smith of Nebo, Ill. For a chronic catarrhal enteritis.

I believe Wm E. Smith now is and has been for many years afflicted with this chronic diarrhoea to the extent of being partially & at times totally incapacitated for manual work.
