His description is : 5 feet 5 inches, blue eyes, brownish hair, light complexion, oval face, high forehead, large mouth, medium chin and nose. Unfortunately no photo accompanies the application.

1910 - living at 393 Beacon St in Boston Mass with his wife Kate P who is 38 years old, born about 1872 in Pennsylvania, her father was born in New Jersey and her mother was born in Pennsylvania. They have been married for 6 years (1904 ?) and have no children. His occupation is physician - anesthesia.
1911 - on his passport application dated 7 July 1911 his description is different from the 1894 application : 5 feet 5 inches, gray eyes, brown hair, medium complexion, full face, high forehead, large mouth, a brown beard on his chin and a broad nose. Unfortunately no photo accompanies the application. He is now a physician living at 483 Beacon St in Boston Mass.
