David Jon Ward, 27, died Thursday, March 3, 1994, in the North Central Human Services home, Forest City, after a long illness. Services: 11 a.m. Monday, Good Shepard Lutheran Church, Decorah. Burial: Haney Ridge Cemetery, Barnum, Wis. Friends may call from 5 to 8 p.m. Sunday at French & Balik Funeral Home and after 10 a.m. Monday at the church.

Survivors include his parents, Larry and Patricia of Decorah; a brother, Mark of Richmond, Minn.; and a sister, Dawn Tollefson of Decorah.
