Jan Kluzak was born on Thursday, May 19, 1877 in Cleveland, Ohio, the son of John and Mary (Pekarek) Kluzak. He was their second child.

In 1883, the parents and their three children, Anna, Jan and Ottokar moved to a farm in Tabor township, Polk County, near Tabor, Minnesota.

John attended school in the Tabor community.

John passed away on Tuesday, March 20, 1895 at his home near Tabor, Minnesota.

John was survived by his parents; 2 sisters, Anna (Joseph) Machal and Rose Kluzak; five brothers, Arthur, Joseph, William, James, and Edward Kluzak; two nieces, Mary Machal and Anna Machal.
