A Native American from Phoenix, AZ. Crew Chief of a UH-1H helicopter from A Company, the "Comancheros," 101st AVN Battalion, 101st Airborne Division. While on a combat assault inserting ARVN Marines near Hue, Thua Thien Province on May 16, 1971, helicopter took fire, lost power and crashed. Also lost were Aircraft Commander CWO Craig Lee Farlow, Pilot CPT Joseph Paul Nolan, Door Gunner SP4 Timothy John Jacobsen and an unknown number of South Vietnamese Marines. Listed as MIA, declared dead on May 17, 1972. Wall location: 01W-026.

After several unsuccessful searches of the crash site, the body of SGT Jacobsen was recovered on June 7, 1995 and identified March 24, 2008. The bodies of the other crew members have not been recovered.
