According to his Compiled Military Service Record (CMSR) on file in the National Archives and Records Administration (NARA) at Washington, D.C., John Calvin Lewis enlisted in the 123rd Infantry Regiment of Indiana, Company C, October 22, 1863, at Clinton, Indiana. He was a 31-year-old farmer born in Sullivan County, Indiana, and was 5 feet 8 inches tall and had blue eyes and brown hair. The 123rd mustered in on December 10, 1863, at Terre Haute, Indiana. John was promoted to Corporal June 15, 1865. He was mustered out on August 25, 1865, at Lexington, North Carolina.

OBITUARY
LEWIS, John C. of Wabash Township died August 29, of brain fever.* He was born in Crawford County on October 20, 1832, and moved with his father in 1854 to Big Creek, his father died in 1854. In 1859 he married Mary Ann Thompson, and in 1863 fought in the war. The funeral was Friday and was buried in Thompson Cemetery. ("Clark County Herald," September 10, 1878, Tuesday, pages 4 and 8.)

*The cause of death was listed as "Typho Malarial fever" in the "Register of Deaths," Clark County, Illinois, Death Index (1877-1911), page 13. The "Date of Report" was recorded as Sept. 1878.
