Azariah was the son of Samuel Alwood Hawkins. His home was Stony Brook. He married Charlotte "Charity" Terry; they had two children, Nancy Jane and Franklin Azariah. A school teacher before the Civil War, he enlisted on September 26, 1862 at Brookhaven, New York. He served with Company F, 159th Infantry Regiment New York. He died at the Army Hospital in Baltimore, Maryland.
