Annette "Cookie" Smith, age 56 of Bradfordsville, passed away Sunday, April 25, 2021 at Ephraim McDowell Regional Hospital in Danville. She was born January 13, 1965 in Bradfordsville to the late Herbert Ray and Elvena Mae Price Simpson. She is also preceded in death by her husband, John T. Smith; her brother, Glenn Ray "Boo Boo" Simpson and three infant siblings, Leona Gail Simpson and twins David Ray and Donna Fay Simpson. Survivors include two children, and three grandchildren, Nicole Smith, David Varney and Nicholas Varney. Funeral Services will be held at 11 am Friday, April 30, 2021 at Mattingly Funeral Home, 195 Holy Cross Road, Loretto. Rev. Dan Durham will officiate. Burial will follow in Old Liberty Cemetery. Visitation is 3-8 PM Thursday at Mattingly Funeral Home. Due to Covid-19 restrictions, the funeral home is limited to 60 % occupancy, face coverings are required, and food or drink are prohibited.
