There appears to be some confusion about her maiden name. Death certificate identifies father as a McCollion (and mother as Elizabeth Wayne), though Social Security records list father as Albert McClennan, while her daughter Violet's death certificate says that Ophelia was a McClellan. She appears to have been born in either Sulphur or Choupique ("Shoepeak"), Louisiana on August 5, 1886 or August 11, 1885, depending on whether you believe the death certificate or Social Security records.

She was survived by three sons, Dewey Turpin of Monahans, Mason Elender of Corpus Christi, and Carson Elender of Houston; and five daughters, Mrs. Ruth Cones of Leggett, Mrs. Violet E. Wells [sic] of Corpus Christi, Mrs. Velma Betts and Mrs. Addie Hardie of Houston, and Mrs. Birdie Grider of Lufkin ("The Polk County Enterprise," Livingston, Texas, November 28, 1957).
