He was born Dec. 3, 1952, in Peoria to Carroll Lee and Helen Marie Goble Cook Sr.
His parents and one sister, Judith Green, preceded him in death.
Surviving are four brothers, Carroll L. Cook Jr. and Joseph Cook, both of Spring Bay, and Gerald W. (JoAnn) Cook and Robert L. Cook, both of Pekin; and five sisters, Sharon (Rick) Drake of Pekin, Ora "Cookie" (Guy) Fisher of Anchorage, Alaska, Beverly J. (Lawrence) Huff and Debbie Goodin, both of Manito, and Rebecca (Richard) Dringenberg of Wheatland, Mo.
Tommy was a self-employed painter in the Peoria area for several years.
He served in the United States Army.
He had a soft heart for children and animals and was active in the Crusaders for Kids organization. He also enjoyed fishing and car racing.
A graveside service will be at Lakeside Cemetery in Pekin.
