Phyllis graduated from Fietshans High School, Springfield, Illinois and earned her bachelor's degree from Capital Area School of Practical Nursing, in Springfield.She was employed as a Registered Nurse for over 20 years. Phyllis was preceded in death by her parents, Earl Lowell and Mildred Leona (Shelly) Jack, brothers, Richard Jack, Donald Jack, Ajax Jack, William Jack, Robert Jack, and Dennis Jack; and sisters, Glenda (Jack) Chapman, and Patricia (Jack) Beaver.
She is survived by her husband, Willie (Fox) Carroll of Springfield; son, Brian Carroll of Springfield; daughter Kimberly (Carroll) Kurniskie; grandchildren Jessica Kurniski, Tony Carroll, and Joey Carroll, all of Springfield; several great-grandchildren; brothers, Harold (his wife, Lorraine) Jack of Springfield, Illinois; Ronald Jack of Pensacola, Florida; Terry and (his wife, Rose) Jack of Dawson, Illinois; Dwayne Jack of San Diego, California; sister, Cynthia Jack of Peoria, Illinois, and many nieces and nephews.
Graveside services will be held at Roselawn Memorial Park, 1735 South Camp Butler Road, Springfield, Illinois on Thursday, September 19, 2019 at 10:00 AM. The Celebrant Judy Woerner officiating.
