Note: the gravestone for Samuel was in perfect condition when this cemetery was visited in 1896. The stone has since deteriorated or disappeared.

Samuel was the son of Aaron and Dorothy Forman of Hempstead and Oyster Bay, Long Island. Samuel was likely born at Hempstead, and moved with his parents to Oyster Bay when still young. He was married about 1685 to Mary WILBUR, the daughter of Samuel Wilbur, Jr. and Hannah Porter of Portsmouth, Rhode Island.

As a young man Samuel moved to Monmouth County, New Jersey with his brothers Thomas and Alexander, and on 29 Nov 1695 he was commissioned the high sheriff of Monmouth County by Governor Andrew Hamilton. Samuel deeded land to his son Aaron in 1719, and in a deed dated 16 Jan 1725, Samuel Forman "late High Sheriff of Monmouth Co." conveyed certain lands in that county to his son Captain John Forman.
