Death Certificate lists his name as "ALVARADO", which is his mother's maiden name, so I think his name on the plaque as "Alvardo" is incorrect.

Mr. Valenzuela, 50, was born in Tombstone, Arizona. He was killed Friday and died in seconds from a cerebral hemorrhage and trauma in a construction accident at Glen Canyon Dam, Page, AZ. Survivors include his wife, Carmen; two daughters, Mary Lucy of Glendale, AZ and Mrs. Dora Moya of San Jose, California; two sons, Frank and Tony of Glendale; a brother, Rosario of Glendale; his parents, Pablo & Marie Lucy (Alvarado) Valenzuela of Glendale and natives of Mexico; and four grandchildren.

Dam Worker Hurt In Fall Now In Critical Condition
PAGE - Officials at the Page Hospital said yesterday that Thomas Sciortino's condition is critical. The 32-year-old cement finisher was injured in a fall at the Glen Canyon Dam on Friday. Several spectators on the bridge watched as one man was killed and two were injured in the construction accident involving Pablo Valenzuela, 50, of Glendale, a carpenter who was killed when he was knocked off a scaffold by falling equipment and fell 350 feet to the base of Glen Canyon Dam following a "hoist cable break". Richard Heuer, 27, a cement finisher, was working on a form and was able to ride it to the bottom of the dam. Heuer was knocked off the dam by the scaffolding. He fell about 300 feet, but was able to walk to the ambulance. He was listed in good condition.

KANAB (UP) — A construction worker at the Glen Canyon Dam project was crushed to death, becoming the second fatality at the construction site, officials reported today. Buford Persinger, 38, Spokane, Washington, was killed when crushed by a slab of falling rock on Monday. He was working on the powerhouse access tunnel for Gibson-Roberts Construction Co., subcontractors. He was killed while operating a drill from a jumbo rig on the river-level portal of the tunnel. The first fatality at the damsite was recorded Aug. 29 when Robert Frazier, 38, Fredonia, AZ, was killed by falling rock."
