He was born on July 25, 1984, to Terry Puckett and Janice Hodgson. His parents survive. Also surviving are his daughter, Ava Jersy Puckett; sister, Amber Raimondo of North Pekin; brother, Aarron Puckett of Pekin; stepfather, Lonie Latham; paternal grand parents, Don Puckett and Janet (Jerry) Palmer; and Maternal grandfather, George Hodgson.
Chad liked to mushroom hunt and fish. He had a heart of gold and a great smile.
Graveside services will be at Trivoli Cemetery in Trivoli, IL.
