Hazel Perkins Smith, Orem, died May 29,1977, at Utah State Hospital. She was born June 6, 1913, in Bedford, Wyo. She married Lynn Vann. They were later divorced. She married Orvil Vann. They were later divorced. She married Duncan Smith. He later died.

She has lived in Utah for 18 years and was a member of the LDS Church. Survivors include a son and three daughters, Mrs. Merrill (Zella) Anderson, Orem; Mrs. Jerry (Maxine) Hazzard, Saudi Arabia; Mrs. Alfred (Lorna) Olson, Minneapolis, Minn.; Allen Vann, Boise, Ida. ; 10 grandchildren. A funeral is scheduled Wednesday at 11 a.m. at Utah State Hospital. Friends may call at Walker Mortuary, Provo, Tuesday between 6 and 8 p.m. and Wednesday at the hospital chapel. Burial will be in Salem City Cemetery.
