Jack was born to Byford and Viola (Pellhum) Bolen. He married Muriel Elaine Grove on June 29, 1969, in Bartonville.

Surviving are his wife of 43 years, Pastor Elaine Bolen; two sons, David H. (Monique) Bolen and Aaron P. Bolen; four granddaughters, Taylor, Savannah, Lalaina and Rebekah; and his mother, Viola Bolen.

He was preceded in death by his father and grandparents.

Jack was a Navy career man, serving 21 years three months and 15 days. He was a Vietnam veteran and served on the USS Saratoga, USS Constellation, USS Enterprise and USS Ranger. He was awarded and earned the Navy Commendation. He was serving on the admiral staff as senior chief petty officer when he retired on July 31, 1988.

After his Navy career, Jack worked for the Department of Employment Security with the state of Illinois as a field office supervisor. Jack was an avid fisherman and enjoyed painting and wood burning.

Cremation will be accorded. Inurnment will be in Parkview Cemetery Chapel of Peace.
