Hazel McCollum Was Home Nurse
Hazel Emma McCollum, affectionately known by many as "Grandma Mac," 89, died Tuesday night July 5, 1988, at the Countryside Health Care Center, where she had been a resident since February 1987.
She was a woman known for her devotion to her children and grandchildren, as well as the community, and for her concern and care of others. For 25 years, she lived in various homes and provided care and companionship for the elderly until her retirement in 1978.
She attended the Nazarene Church in Frankfort, Ind., and enjoyed needlework.
Born Nov. 17, 1898, in Chatsworth, Ill., she was the daughter of Charles and Mary Emma (Wenger) Perkins. She was reared and educated in Chatsworth, where she resided until her marriage March 6, 1920, to Burel P. McCollum. They were later divorced.
During their marriage, the family lived in Piper City and Gilman, Ill., prior to moving to Frankfort in 1950. She moved to Lebanon in 1981.
Surviving are children Mary Kincaid Williams of Lebanon; Charles William McCollum of Sebastian, Fla.; and Duane Richard McCollum of Peoria, Ariz.; 16 grandchildren; 27 great-grandchildren; and four step-great-grandchildren.
She is also survived by a brother, Roy Perkins of Chatsworth, Ill.; sisters Marietta Albee of Fairbury, Ill., and Myrtle Hamilton of Park Ridge, Ill. She was preceded in death by brothers Russell, Archie and Francis Perkins and a sister Mabel Perkins.
Services are arranged for 10 a.m. Saturday at Myers Chapel of Memories with burial to follow in Greenlawn Cemetery, Frankfort. The Rev. John Lietzel will conduct the funeral. Visitation at Myers Mortuary is after 4 p.m. Friday.
