On this 15 day of April A.D. one thousand eight hundred and 93 personally appeared before me Notary Public LUCINDA SMITH aged 55 years, who, being duly sworn according to law, makes the following declaration in order to obtain the Pension provided by Acts of Congress granting pension to widows: That she is the widow of BALLARD SMITH, who enlisted under the name of BALLARD SMITH at Harrodsburg, on the 22nd day of November 1861 in Co K, 19 Regt Inft Vol as a private in the war of late Rebellion who was granted Pension for Right ingurnal hernia (Complicated) and Rheumatism and resulting disease of heart. And from the above disease said BALLARD SMITH DIED ON THE 19 DAY OF MARCH 1893 who bore at the time of his death the rank of Private that she was MARRIED UNDER THE NAME OF LUCINDA BROCK TO SAID BALLARD SMITH ON THE 27 DAY OF SEPT 1883 by Judge Brown at Wm. Routins there being no legal barrier to such marriage; that applicant were previously married & HER FORMER HUSBAND DIED MARCH 16, 1870. "THE DECEASED FORMER WIFE DIED April 19, 1882."
