Born March 30, 1892, Vernal, to John H. and Martha Agnes Nuttal Cook. Married Violet Beatrice Hodgkinson July 16, 1910, Vernal. Later solemnized Salt Lake Temple, Church of Jesus Christ of Latter-day Saints. Carbon County resident 35 years.
