5 Feb 2010 - At some point in his life, Phil changed the spelling of his last name from "Muschalik" to "Mushalik". No information as to why he did this has been found to date.
10 Mar 2023 - Still no source showing when Phil dropped the "c" in his surname. It seems to be sometime soon after his marriage to Carol.
16 May 2023 - On 18 Feb 1965, Phil officially dropped the "c" in his birth surname of MUSCHALIK. See birth correction document in his folder in "My Family History" folder on my computer. He actually had been using the MUSHALIK name for many years.
Carol & Phil did not have any children.
