The monument is a marble obelisk set on a concrete base. It is approximately 11 feet tall.

The inscription honoring Governor Tiffin is on the south face of the obelisk. Immediately below that inscription is a separate one memorializing his son, Dr. Edward P. Tiffin (1822-1853). The east face honors Governor Tiffin's wife, Mary Porter (d. 1837, age 48). The family name, TIFFIN, is on the base of the east face. The west face has the name COOK in a similar position (the married name of Governor Tiffin's daughter). The north face is blank.

Mary Porter was not Tiffin's first wife. He was originally married to Mary Worthington (1762-1808), sister of Thomas Worthington, another early settler of Chillicothe, who also served as Governor and United States Senator. Mary's grave stone is some distance away in the Worthington plot.

While Governor Tiffin died in 1829, the monument dates from a later period. A brief article in the October 21, 1866 edition of the New York Times tells of the recent erection of a monument of pure Italian marble in his honor at the cemetery in Chillicothe. It's reasonable to believe that his son's family raised the monument to honor the Governor, his wife, and his namesake son. What became of the original marker, if it ever existed?

The marble is heavily eroded, and large parts of the inscription are only faintly readable, with some portions illegible. The 1866 New York Times article carried a copy of the inscription, although there are some apparent differences in abbreviation, punctuation, capitalization and the segmenting of words. Between what was left on the stone and the newspaper article, I've reconstructed the following:
