Death Record lists her name as ADA S. PUFFER. Her death record erroneously says that she was born in Dexter, Cambridge, and uses the initials of MA, instead of ME. Lists her father as Wilson and mother as Lucy. She was a twin to Addie Puffer, whose birth record is recorded on 06 MAY 1864 in Brewer, Maine, and erroneously says that she was born stillborn. Both of these twin sisters appear in the 1870 U. S. Federal Census in Brewer, Penobscot Co., Maine, with their parents, and both are listed as age 6. They were the second set of twins born to Wilson Puffer and wife Lucy.
