DAY, Samuel M. Co. I, 4th VA. Inf. Enlisted: 1/17/63, Where: Camp Winder, Richmond, Va. C.M.R.: Jan. to Apr. 1863 present. General Hospital No. 12, Richmond, Va. Surgical Case: Samuel Day, May 1863, Age 18, Farmer. Wounded on the 3rd of May by a ball fracturing both bones of leg near the knee joint. Amputation was performed on the 4th at the lower third of the thigh. Admitted into Hospital 12 Gen. Health much impaired and stump unhealthy. Died Jun. 9, 1863 from amputation lower 3rd of left thigh. Wounded at Chancellorsville 5/3/63.
