Appointed Confederate Army Assistant Surgeon by President Jefferson Davis on April 27, 1861. Captain Joseph J. Holt accepted his commission on May 14, 1861 and was assigned to the 2nd Mississippi Volunteer Infantry Regiment. Confederate Secretary of War, James A Seddon, promoted Joseph J. Holt to Surgeon on June 1, 1864 with a date of rank retroactive to November 1, 1863. During his years of service, the 2nd Mississippi Infantry fought the battles of 1st Manassas, Seven Pines, Gaines Mill, Malvern Hill, 2nd Manassas, South Mountain, Antietam, Suffolk, Gettysburg, Wilderness, Spotsylvania, Cold Harbor, Weldon Railroad, Petersburg and Hatcher's Run. On April 9, 1865, Major Joseph J. Holt was paroled at Appomattox Court House with the 2nd Mississippi Infantry Regiment, Joseph R. Davis's Brigade, Heth's Division, 3rd Corps, Army of Northern Virginia.
After the the Civil War, Joseph Johnson Holt, M.D. established his medical practice in New Orleans and was appointed a professor of obstetrics at the New Orleans School of Medicine. During his service as President of the Louisiana State Board of Health from 1884 until 1888, Dr. Holt became renown for work to prevent smallpox and support for tougher marine vessel quarantine procedures to prevent the spread of yellow fever throughout Louisiana and up the Mississippi River.
