Myrtle was born in Roosevelt, Duchesne County, Utah to T. Golden and Florell Marx Collins.
Her schooling took place in the Uintah Basin in the Roosevelt schools. Upon graduation, she attended Utah State University and graduated Magna Cum Laude.
She was an active member of the LDS Church and served a 2-year mission for her church to Brazil. She served in many positions for her church. She was a Relief Society teacher, visiting teacher and Gospel Doctrine teacher, among others.
In 1958, Myrtle married Orlan Cook in the Salt Lake LDS Temple on Apr. 21. Orlan was a farmer and commissioner for Uintah County. Myrtle helped on the farm and was devoted to her husband and children. She had four children, Judy, Peggy, Brent, and Grant, who were the love in her life.
When Orlan had an accident crushing his hip, she was his caretaker and was devoted to caring for him and their family. As he improved, Myrtle began working in Todd Elementary School and working on her Masters Degree. She taught in Todd Elementary School for thirty years teaching the children she loved. She was Teacher of the Year in the Uintah School District in 1991, and was one of the first 10 recipients of the Huntsman Excellence in Education , awarded by the governor in 1993.
She loved life and was always optimistic. She enjoyed working in the yard with flowers in her spare time. She developed a congestive heart condition, which curtailed work in the yard, but was able to continue teaching until her passing on Oct. 26, 1997. She passed away at home, and funeral services were held in the Ballard LDS Ward Oct. 29, 1997. Burial was in the Roosevelt Memorial Park.
She was survived by her husband, Orlan of Ballard; sons and daughters, and spouses: Judy and Lynn Cornia, Riverton, Wyoming; Peggy and Tracy Killian, Roosevelt; Brent and Toni Cook, and Grant Cook, all of Ballard; 9 beloved grandchildren; brothers and sisters, Boyd Collins, Clyde Collins, Mrs. Earl (Mary) Taylor, Allen Collins, Mrs. Ricky (Gaye) Berg, Mrs. Duane (Kaye) Frisby, and Thomas Golden Collins, Jr.
She was sorely missed by all family and friends.
