First Lieutenant Forrey served with the 2525 Base Unit at Liberal Army Airfield, Kansas. He was killed attempting to bail out of RB-24E Liberator #41-28559 after the bomber which he was piloting caught fire in flight, nine miles north of the airfield. 2nd Lt. Charles H. Post also perished.

He was the son of George L. and Hazel Avis Forrey.
