Page 686 of Samuel P. Bates's History of the Pennsylvania Volunteers, 1861-1865 shows a Corporal Henry D. Conrad in Company D, 151st Pennsylvania Volunteer Infantry Regiment. Company D was recruited in Juniata County by George Fisher McFarland, Principal of the McAlisterville Academy, the 151st's first commander at Gettysburg until his wounding and Superintendent of the Pennsylvania Soldiers Orphans Schools.

Corporal Henry D. Conrad: Promoted from Private to Corporal on November 1, 1862. Captured at Gettysburg on July 1, 1863. Mustered out with the Company D, 151st PVI on July 27, 1863.

If this is the same soldier as this memorial, then your grandfather was a hero of Chancellorsville and Gettysburg. A small number of men mustered out of Company D, 151st PVI and later reenlisted in other Union Army units. Check the history of his "Heavy Artillery" unit, most were artillery in name only. If he reenlisted in 1864 the army probably issued him a muzzle loading musket and put him in an infantry unit that was recruited using the more glorious "Heavy Artillery" name.
