JERALD MARK BREZOVACKI, 50, passed away Friday, Oct. 14, 2011 at his resi dence in Convoy, Ohio. Born in Chicago, Ill., he worked as an MPC Trunking Technician with Frontier. He was a U. S. Air Force veteran. Surviving are his wife, Donna Brezovacki of Convoy, Ohio; father, Allen Gleason of Fort Wayne; mother, Shirley Hord of Fort Wayne; daughter, Jamie (Raymond) Brandt-Almy of Warsaw; sisters, Laura (Jay) Leasa of Eldorado, Wis. and Pam (Larry) Hummer of Huntertown; brothers, Chris Gleason and Scott Gleason both of Fort Wayne. Funeral Service is 10 a.m., Wednesday, Oct. 19, 2011 at D.O. McComb and Sons Foster Park Funeral Home, 6301 Fairfield Avenue with Pastor David Harold officiating. Calling is from 2 p.m. to 4 p.m. and from 6 p.m. to 8 p.m. Tuesday, Oct. 18, 2011 at the funeral home. Burial in Rochester Cemetery, Cecil, Ohio. Memorials may to the Wounded Warrior Project.
