NORFOLK, Va. - A National Guard pilot was misinformed about his altitude before his jet collided with a private plane carrying seven people off the North Carolina coast, according to an Air Force report.
The report, however, contains no recommendations for disciplinary action against any of the military or civilian air controllers, or the military crews involved.
The civilian plane's pilot - Henry Tiffany, 47, a Charlottesville lawyer - and his six passengers were killed Jan. 9 when his twin-engine Beechcraft 55 Baron, flying to Norfolk from the Bahamas, crashed into the Atlantic Ocean about 40 miles southeast of Jacksonville, N.C.
The other victims were Edward Furniss, 20, Jo Al Kohl, 23, Richard Schlimgen, 20, James Slacke, 18, and Robin Graham Streib, 24, all of Virginia Beach; and Elizabeth O'Mahony, 24, an Irish housekeeper employed by Tiffany. No bodies were recovered.
One of two National Guard F-4 Phantom jet fighters sent to intercept Tiffany's plane in resrticted military airspace over the Atlantic apparently made a left turn and crashed into the private plane.
The Air Defense Command gave the military pilot the wrong altitude readings for both himself and the civilian plane, the report said, and moments later the military pilot took a left turn into the civilian plane.
Personnel at the Federal Aviaition Administration's Flight Control center in Washington and Fort Lee were cited for having been "slow to react or acted improperly in the process of identifying the unknown aircraft."
Transcripts of the conversations among the air controllers and pilots show that the Phantom jet had continued its pursuit for seven minutes after Tiffany's plane had been identified because an FAA controller had not properly notified the Air Defense Command.
