PATMOS - The Formby, 77, of Patmos, died January 20, 2010, peacefully at home surrounded by family. He was born November 4, 1932, in Lewisville.

He was the son of the late Narvel and Thelma Formby. He was a member of Central Church of Christ and was a 1950 graduate of Lewisville High School. He was a veteran of the Korean War and was a retired poultry farmer.

He was an outdoorsman who loved working on his home, farm, and tractors and he also enjoyed working on older model cars. He and Ava Nell Honeycutt Formby were married for 57 years and he was a loving, devoted husband, father, grand and great-grandfather.

He was preceded in death by his parents and two brothers, Billy and Travis Joe Formby.

Survivors include his wife, Ava Nell Formby, of Patmos; two sons, Duane and Belinda Formby and David and Vanessa Formby, and one daughter, Cheryl and Gary Lee Banks, all of Patmos; eight grandchildren, David and Evelyn Formby, Steve and Jennifer Formby, Aimie and Kyle Barnett, Jamie Smith, Fallon and John Scoggins, Amber and Jeremy Stroud, Chelsey and Dustin Mills, and Chaley and Jason Porter, and 10 great-grandchildren, Luke, Colt, Brooklyn, Jacob, Jessie, Madison, Westin, Maverick, Anna, and Whitt.

Services will be held at 2 p.m. on Saturday, January 23, at Smith Funeral Home Chapel in Stamps with Minister David Lynn Formby and Harold Freeman officiating.

Burial will be at Mt.

Nebo Cemetery in Patmos, under the direction of Smith Funeral Home. Visitation will be 6-8 p.m. Friday, January 22, at Smith Funeral Home in Stamps.
