Blake was the son of Johnny Douglas and Nancy S. (Jones) Wallace. He passed away at Lake City Medical Center in Lake City, CO. He had attended Lane School in Atoka County and was a member of the Farris Church of Christ. He was Vice-President of Southeast District 4-H, President of Lane 4-H Club, and 1997 First Place Winner of the Oklahoma Academy of Junior Science. He is survived by his parents, Doug Self and wife Marianne of Stringtown, OK; Nancy Wallace and husband Eric of Lane, OK; sisters, Kaci Self of Atoka, OK and Destiny Self of Stringtown, OK; maternal grandparents, Mary and Dan Isom of Kiowa, OK, Udell Jones of Lane, OK and Leon and Vondell Jones of Lane, OK; paternal grandparents, Johnny and Zelma Self of Stringtown, OK, Ed and Joan Wallace of Antlers, OK and Ty and Dean Bowman of Canadian, OK; great grandparents, Lavada Hammond of Farris, OK, Verdia Mae Jones of Atoka, OK, Florence Lemons of Kiowa, OK and Flora Isom of Daisy, OK. He was preceded in death by his great grandparents, Earl T. Jones on June 16, 1994, Robert Hammond, Ernest Lemons on Nov. 22, 1993, Adolphus Isom and Jack (Jan. 20, 2000) and Mary Self. Services will be Sunday, Oct. 12, 1997 at 3 p.m. at Lane School Gymnasium in Lane, OK. Interment will be at Butler Cemetery.
