Jack is the son of Boyd Formby and Maggie Pierce Evens Formby. Jack married Florence Marie Bennett on 22 May 1941 in Crawford, Arkansas. On 29 August 1940 Jack enlisted in the US Army Air Corps World War II as a private for the Philippine Department. He was a member of the 115 OBSN. SQ 69., AC. Air Force. Jack was killed in a airplane crash while in service at Army Air Base in Laurel, Mississippi.At his death in 1943 he was a Sergent.
