This is my Uncle -age 3 month old.

Illinois Statewide Death Index, Pre-1916 & death certificate show name as Walther H. (Index) & Walther Henry Mushalik (certif).

16 Dec 2011-although Walther's death certificate indicates that he was buried in Forest Home Cemetery, Find A Grave photo volunteers have been unable to find a gravesite. Forest Home Cemetery has no record of the burial.

14 Jul 2015-Forest Home Cemetery indicates that there is a burial plot (added here) for Walther H. Musbalek but no marker. I will inquire further as to the cost of a marker for Walter with the correct spelling of surname.
