Having actually gone "Missing" on the above date, he was not officially declared by the military as being dead until February 9, 1946.

David served as a Lieutenant Colonel & Pilot on P-38J #43-28516, 9th Fighter Squadron, 49th Fighter Group, U.S. Army Air Force during World War II.

He resided in Ohio prior to the war.

David took off, with 18 other P-38's, in P-38J #43-28516 from Hollandia Airfield, Jayapura Regency, Papua Province, Indonesia on an escort mission for B-25's over Babo, West Papua Province, Indonesia.

On this mission they were attacked by Japanese Zeros and LTC Campbell is believed to have been shot down as he never returned to his base after the mission.

David was declared "Missing In Action" on this mission during the war.

He was awarded the "Distinguished Service Cross", "Distinguished Flying Cross with 1 Oak Leaf Cluster", Air Medal with 2 Oak Leaf Clusters, and the Purple Heart.

The President of the United States takes pleasure in presenting the Distinguished Service Cross to David A. Campbell (O-392746), First Lieutenant (Air Corps), U.S. Army Air Forces, for extraordinary heroism in connection with military operations against an armed enemy while serving as Pilot of a P-38 Fighter Airplane in the 35th Fighter Squadron, 8th Fighter Group, FIFTH Air Force. During the period from 1 to 29 May 1943, Lieutenant Campbell shot down a total of three enemy aircraft in aerial combat. First Lieutenant Campbell's unquestionable valor in aerial combat is in keeping with the highest traditions of the military service and reflects great credit upon himself, the 5th Air Force, and the United States Army Air Forces.
