His death certificate gives his middle name as "LOOSE". The informant was his daughter Mrs. Oliver Nutter, (Gertrude M.). A number of online trees give his dob as 12 Nov 1849, although his death certificate gives 12 Dec 1849. Cause of death: accidentally killed by Nickel Plate passenger train.

"Machine Crashes Into East Bound Nickel Plate Passenger Train Sunday Afternoon in Outskirts of Leipsic
Moffitt was instantly killed. Mrs. Moffitt, his wife, was seriously injured and Mrs. Ruth George, their daughter, was hurt when an automobile in which they were riding hit a Nickel Plate passenger train at 4:30 o'clock Sunday [July 9] afternoon in the outskirts of Leipsic.
The Moffitt machine had just crossed the track on which the freight was standing when the passenger train which was going 40 miles an hour shot out from behind the box cars directly in front of the automobile, which was going at a good rate of speed, also. The machine crushed into the passenger train and was instantly demolished and one if it's occupants killed and the other two hurt.
Mr. Moffitt was one of the best known farmers in Hancock county. His farm is located a half mile south of Moffitt Station, which was given his name by the Moffitt family. He was a son of Mr. and Mrs. John J. Moffitt who was one of the early pioneers of Hancock county. He was an uncle of A. Putnam Sandles, of Ottawa, O., who is a candidate for the Democratic nomination for governor. His niece married Mr. Sandles.

One son and a daughter are dead."
