Jack Condon's birthplace has variously been recorded as "Charleston, South Australia" or Charlestown, South Burnett Region, Queensland". His death certificate, signed by his father, states clearly he was born in "Charlestown (Charleston), Etheridge" and that he was "Never out of Queensland (except War)". Charleston was renamed Forsayth in December 1910.
