Bleech was killed in a plane crash in the Philippines. He was originally listed as Missing in Action or Buried at Sea; his name is inscribed on the Tablets of the Missing at West Coast Memorial, San Francisco, CA.

He was awarded an Air Medal & Additional Navy-Marine Corps Awards, also awarded the Navy Cross & Distinguished Flying Cross posthumously, for heroism and extraordinary achievement while participating in aerial flight as pilot of a carrier-based torpedo plane in an attack on a large enemy convoy in the vicinity of the Marianas Islands on 12 June 1944.

In the face of enemy anti-aircraft fire, he so skillfully and courageously delivered his attack as to score direct hits with rockets and to cause excessive damage with bombs on an enemy merchant vessel which, when last seen, was burning furiously and abandoned.

The President of the United States of America takes pride in presenting the Navy Cross (Posthumously) to Lieutenant, Junior Grade Otto Ross Bleech (NSN: 0-278304), United States Naval Reserve, for extraordinary heroism in operations against the enemy while serving as Pilot of a carrier-based Navy Torpedo Plane in Torpedo Squadron FIFTEEN (VT-15, attached to the U.S.S. ESSEX (CV-9), in action against the enemy Japanese Fleet during the Battle for Leyte Gulf, on 24 October 1944. Skillfully flying his plane through extremely intense and accurate anti-aircraft fire Lieutenant, Junior Grade, Bleech approached to close range and obtained a torpedo hit on an enemy heavy cruiser causing certain damage. His determination and courage were at all times in keeping with the highest traditions of the United States Naval Service.
