Ellsworth served as a Technician Fifth Grade in the 508th Parachute Infantry Regiment, U.S. Army during World War II.

He resided in New Castle County, Delaware prior to the war.

He enlisted in the Army on October 30, 1942 in Camden, New Jersey. He was noted as being employed as a Carpenter and also as Single, without dependents.

He "Died Of Wounds" received during battle and was awarded a Purple Heart.

At the families request, his state of birth was added to his headstone (California) instead of his state of residence (Delaware).
