Collista Adelaide Clark Perkins was born in Allamakee county October 20,1858, daughter of John r. and Maria Bray Clark. She passed away Feb.19,1938 at the home of her daughter Edna Bollman with whom she lived for the past 15 years.
On September 17,1884 she was united in marriage with James Perkins who passed away at their home in April 1914. James and Collistia were the parents of one daughter Mrs. Edna Bollman. Mr and Mrs Perkins spent all their life on a farm near Forest Mills and many years ago united with United Brethern Church at that place. She is survived by her daughter and son-in-law, Edna and Ira Bollman, seven grandchildren, and one great grandchild.Also a brother Delbert of Ashville, New York.
Funeral service was held at Schultz Funeral home in Monona. Services were conducted by the Rev.G.W. Turner. Interment was in Smithfield Cemetary By the side of her Husband
